Highly skilled geriatric social workers provide personalized aging consultation and assessment for older adults and their families, including Holocaust survivors. Staff evaluate social connections, family support systems, medical and cognitive challenges and home safety, and provide resources as needed. They also help families explore home care options and other supportive services. Working closely with clients and their families, staff create a personalized care plan, outline recommendations, and connect them with resources to address specific challenges and support aging with dignity and independence.
