Seniors At Home – Care Management/Healthcare Advocacy
Professional care managers assist individuals and families in navigating health-care needs. Care managers advocate on clients’ behalf and help navigate complex situations. They ensure clear communication among family members, doctors, caregivers and other health-care providers, and provide the information and support needed to make informed health-care decisions and enhance independence and quality of life.
Care managers offer one-time consultations or ongoing care management, and they work collaboratively to develop and implement individualized care plans. They are available for consultations at home, in residential facilities or in office settings.
A division of Jewish Family and Children’s Services, a nonprofit organization that has cared for the community since 1850. Serves San Francisco, the Peninsula, Marin and Sonoma counties, and welcomes seniors and families regardless of religion, background, sexual orientation and gender identity.
