The Center for Dementia Care provides specialized in-home support for people experiencing Alzheimer’s or other forms of dementia, as well as practical and emotional support for their families. The innovative approach centers on the strengths and abilities of each individual, with individualized care plans designed to support quality of life, help prevent fluctuations in mood and behavior, and honor each client’s unique identity. The professional team includes social workers, care managers, caregivers and a staff neurologist.
